I collaborate with the Intake Coordinator, the Office Assistant, and the Providers in practice to ensure that your care needs are and to ensure a smooth work environment. Please call and ask me if you have questions regarding your care at the practice. I will assist you with billing-related questions ( copays, account balances, disputes, etc.). Also, please call and ask for my assistance if you have any complaints about our services. While in coordination with the Assistant Clinical Director and the Clinical Director, I will promptly address your concerns and complaints and get back to you promptly. Diana Palmyra. Intake Coordinator

Hello! My name is Diana Palmyra. I’m the Intake Coordinator at Assured Hope Community Health, LLC. I will gladly assist you with the intake process to initiate services with the practice. My role is to ensure a seamless intake process as you become a client here. In addition, I will assist with coordinating your services with the providers, my colleagues, and the insurance as we book your intake appointment. Likewise, I will help you to complete the intake assistance through the OnPatient Portal when you arrive at the practice.

Nivis Nakalema, Office Assistant

Hello! My Nivis Nakalema. I’m the Office Assistant at Assured Hope Community Health, LLC. I will assist you with finalizing the office and the intake process. Likewise, I will help you learn how to use the OnPatient Portal to complete the Intake and follow-up assessments before or after seeing your provider. I work hand in hand with Diana, Natasha, and the rest of the team to ensure your smooth h transition and continuity of care with the practice. I’m the main point of contact to handle medication refill requests, medication prior authorizations, booking and reminding you about your appointment, etc. I’m also responsible for obtaining medical records from your primary physician’s office, specialists, pharmacies, etc.
