This will allow us to determine your mental health needs, and it will also enable us to verify your insurance coverage, etc. When all this data is confirmed, we will send you a welcome email and an intake assessment form via the OnPatient application. When the intake assessment is fully completed via Onpatient, your appointment will be scheduled for an in-person evaluation depending on how soon all documents are completed and verified.
