Thank you for considering Assured Hope Community Health, LLC, for assisting with managing your mental health needs. Please fill out the attached form and send it back to [email protected]. This will allow us to determine your mental health needs, and it will also enable us to verify your insurance coverage, etc. When all this data is confirmed, we will send you a welcome email and an intake assessment form via the OnPatient application. When the intake assessment is fully completed via Onpatient, your appointment will be scheduled for an in-person evaluation depending on how soon all documents are completed and verified.

OnPatient

OnPatient is a secure application that allows you to connect and communicate with your provider from anywhere using your computer, Android, or Apple phone. In addition, the app contains some parts of your health record with Assured Hope Community Health, LLC.

How to access OnPatient

The clinician will create your intake health record when you become a client at Assured Hope Community Health, LLC. HIPAA regulations must apply. The clinician will send you a welcome email with the date and time of your appointment. This email will also contain a link that allows you to sign up on your computer, tablet, or phone. The email will also have the On-Patient app link, which you can download to your phone or tablet.

How to use OnPatient

You must also check-in and confirm your appointment using the On-Patient application before work. You can check in for your appointment 24 hours before the visit. Upon arrival at Assured Hope Community Health, LLC offices, a tablet will be provided for you to sign in and complete the required assessment tools. Before your intake proceeds, On-Patient will provide you with the ability to sign all required Assured Hope Community Health, LLC documents, including, but not limited to, HIPAA laws, our policies, and procedures, financial policies, cancellation of visit policies, etc. For the first visit, On-Patient will allow you to verify and correct your demographics and other portions of your health record, including insurance, referral source, medications, etc. Several assessments and diagnostic assessments must be filled out before the initiation of services with the practice.
